As of Tuesday, June 16, the latest Epic update automatically provides helpful advance care planning resources to eligible patients in their after-visit summary (AVS) following select Medicare wellness visits.

How it works

If a patient is 18 or older and is scheduled for one of the following visit types, their AVS will automatically populate with an advance care planning section complete with a QR code and links to legal document resources:

• Medicare Annual Wellness [44]
• Medicare Wellness Video [1877]
• MyChart Medicare Wellness Vid [1879]
• New Patient Medicare AWV [2411]
• Est Pt, New to Prov Medicare [2468]

If physicians or APPs want to provide information regarding advance care planning and the patient does not meet the criteria above, they can manually attach the advance directive information sheet (PDF) to the patient’s AVS using SmartText or the Attach Reference field.
