Starting on Monday, Jan. 1, 2024, CMS requires hospitals to conduct health-related social needs (HRSN) screenings for all admitted patients aged 18 and over, covering the areas of:

• Food insecurity
• Housing stability
• Transportation needs
• Intimate partner violence
• Utilities

Case management and social workers will handle the screenings, record the responses in Epic, with results accessible on the patient storyboard. The goal is to enhance clinician understanding of patients’ backgrounds, enabling targeted interventions for those at risk and gaining insights into the prevalence and impact of health-related social needs on outcomes.
