
HonorHealth first in Arizona to use real-time OCT imaging in breast-conserving surgery
HonorHealth is the first healthcare system in Arizona to implement the S-Series Optical Coherence Tomography (OCT) device during breast-conserving surgeries. Developed by Perimeter Medical Imaging AI and approved by the FDA in 2021 for use on multiple general tissue types, this advanced technology allows surgeons to examine tissue at the cellular level in real time during procedures.
Why it matters and how it works
- The OCT device provides powerful intraoperative imaging to support immediate surgical decision-making:
- Visualizes tissue microstructures up to 2mm deep in ducts, vessels and glands.
- Produces cross-sectional 3D images at 10 times the resolution of ultrasound and X-ray.
- Offers 100 times the imaging power of MRI.
- Enables real-time assessment, eliminating delays for pathology results.
This technology addresses a critical clinical challenge: nearly 20% of breast conserving surgeries require reoperation due to positive margins found post-operatively. By offering immediate visualization, OCT imaging may reduce reoperation rates and improve patient outcomes.

Sommer Gunia, DO, breast surgical oncologist with HonorHealth Cancer Care in collaboration with the Comprehensive Breast Center of Arizona, has successfully integrated the OCT device into her surgical practice.
“When patients have to return for a second surgery because all the cancer was not removed the first time, they can experience a significant emotional and physical setback. I want to do everything I can to prevent that,” said Dr. Gunia. “Having advanced technologies in the operating room to assist my decision-making in real time is incredibly valuable. OCT imaging allows me to see at a depth and resolution that was previously impossible.”
Improving patient experience
The technology is already demonstrating its impact. One recent patient, a 69-year-old Scottsdale resident, underwent successful breast-conserving surgery using the OCT device and was able to proceed directly to radiation therapy, avoiding additional procedures and treatment delays.
