Accountability in action for our patients and caregivers

Recognizing the ICARE values in the exceptional care that is given to our patients every day!

ICARE: Freyda Velazquez-Perez, MD, exemplifies accountability by putting her patient at the center of what she does and driving for the best outcomes.
In the words of her patient:

“I am very thankful to have Dr. Velazquez-Perez as my primary care physician. Somehow, I wound up with a pretty nasty set of rather debilitating diseases, and the last thing I need is a pro forma, who are you, ten-minute consult. Dr. Velazquez-Perez is always conversant with my medical history and fully able to discuss the various ongoing aspects of my treatment. Perhaps the most valuable trait she displays is an engaged manner; she listens attentively as I try to explain what is going on, symptom changes, test results and what might lie ahead. She attends to the conversation with interest and professional acumen – never distracted by other issues or admin – and offers me a better understanding of my condition. I am always given an explanation as to what medical issues may be at hand and the most appropriate options and alternatives to move forward. I find Dr. Velazquez-Perez a highly professional and approachable physician and value her consultation. A physician that can make eye contact with me, and not the computer, is engaging and relaxing. The most effective doctor in my retinue, she serves as both coach and quarterback as I move along with treatment. My strongest positive recommendation and regard.”
