Patients needing extra help with complex medication regimens? Not meeting medication-related quality measures? Support your patients with a collaborative practice model of care with our ambulatory clinical pharmacy – HonorHealth Medication Management. One-on-one consultations include medication adjustments, lab monitoring, and education, as needed. Make progress with medication management in-between appointments with the referring provider. Referrals are accepted from inpatient and ambulatory settings.

HonorHealth Medication Management is open to new referrals. How we can help:

  • Anticoagulation management: Anticoagulation Center of Excellence distinction
    • Point-of-care monitoring preferred
    • Evaluate for coordination of home testing or outside lab monitoring, when appropriate
  • Diabetes management: Average A1C ↓35% at three-month follow-up
    • A1C >9% (general guideline, not strict requirement)
    • Hospitalization related to uncontrolled diabetes or hypoglycemia
    • Includes insulin management and pumps
  • Heart Failure: Reduce readmissions risk
    • Assist with initiation and titration of guideline directed medications
    • Medication monitoring, adherence and education
  • Other complex medication regimens
