Worried about your older adult patient but don’t have the time or feel comfortable addressing their needs?

Consider a referral to the Geriatric Medicine Consult Clinic. You will remain as the PCP of your patient.

Appropriate reasons for consults are listed below:

• Confusion or memory loss – for this consult reason, please advise a family member/friend also be present for the visit
• Weight loss
• Frequent falls
• Polypharmacy
• Depression
• Advance care planning
• Uncertainty about the ability to live independently
